This is for sale on ebay and was wondering if anyone has seen one of these before and are they a rare item?

Hadn't seen this exact one but I had the modern equivalent when I was in during Desert Storm. I was qualified to drive a fair amount of vehicles. When you were qualified to drive a certain vehicle it was added to the form. I found it recently in my old military stuff.
